The athlete MUST have a CURRENT* sports physical turned into the school nurse by the due dates listed below. Sports physicals MUST be cleared before the first day of tryouts.
- *Sports physicals are valid for 365 days from the first day of TRYOUTS.
- If a student played a previous sport and the sports physical is still valid, the Health History Update Packet from the Athletic Forms website must be completed and turned it into the school nurse by the due dates listed below. This includes an athletics consent form and a Health History Update Questionnaire.
- The School Doctor will review ALL forms submitted for Athletics. It may take up to 2 weeks for athletes to be cleared after forms are submitted to the school. Any physicals handed in after the due dates may not be cleared in time for tryouts
Sports Physical Packet Due Dates:
- Fall Sports – Friday, June 6
- Winter Sports - Wednesday, October 1
- Spring Sports - Monday, February 2
Sports Physical Forms can be printed from the website.
- Sports Physical Forms are also available in the Nurse’s Office, PHS Athletic Training Room, and PHS Athletics Office.
- Any questions regarding sports physicals and health history update forms should be directed to the school nurse.
- During July & August, sports physical packets can be dropped off at PHS - Patton main office - Monday-Thursday, 8 a.m.- 2 p.m.
The athlete MUST take the ImPact concussion baseline test.
- Baseline concussion testing is required for all Piscataway student-athletes interesting in playing a sport.
- All Grade 6, 8, 9, and 11 Students, or anyone new to athletics, are required to test.
- Students can take the ImPACT test at home.

- Testing must be completed prior to your child's sport season.
